Information is sent securely through your phone line and allows us to track your vital signs and other measures of your health status. Your data is monitored by a Village Home Health telehealth nurse. Any significant changes are assessed and, if necessary, your physician is contacted. It also provides your physician with more detailed information that is important for your ongoing care.
