Each and every Village Hospice professional and trained volunteer shares a deeply-felt passion for providing compassionate end of life care. It’s part of their nature to nurture patients, their families and friends during this tender time.

Village Hospice professionals work together as an interdisciplinary team. Our patients, their families and friends are an important part of the care team and work with us as partners, to develop the desired plan of care.

Each team may include, but isn’t limited to:

The team receives oversight and support from our hospice-trained medical director who makes scheduled visits to evaluate the patient’s condition, answer questions and provide detailed care. Our medical director works in partnership with the patient’s primary physician.

Hospice care nurses have specialized training in pain and symptoms management. Their role on the hospice care team is to coordinate the plan of care by assessing and addressing patient and family needs, administering medications, communicating with the physician, teaching comfort measures, and providing direction and education for patients and their families.

The best possible hospice care includes more than medical care. Social workers assess patient and family emotional needs, assist with financial concerns and legal matters, provide direct counseling, assist with end-of-life planning and facilitate communication between the family and community agencies.

These caregivers assist with personal care and comforts that include bathing, grooming and more.

Treating the whole person – body, mind and spirit – includes helping to meet the spiritual needs of the patient and family. Nondenominational chaplains offer support and comfort as requested and desired.

Board-certified music therapists bring comfort and calmness, and help family members connect.

Trained volunteers give their time and talents to offer emotional support, respite time for the family and companionship for the patient.
