  • Our telephones are answered 24 hours a day, 7 days a week. An on-call nurse is available to accept your call and arrange help for urgent needs.
  • Your caregivers will coordinate with your family and your physician to confirm your eligibility for home health services and to implement an individualized plan of care that’s right for you.
  • Medicare or private insurance reimburse for medically-necessary services provided by Village Home Health, based on the level of care and your insurance plan.

Who is eligible for Village Home Health care?

  1. You must be under the care of a primary care doctor and under a plan of care established and reviewed regularly by your doctor.
  2. A doctor must order home health care in a face-to-face encounter and certify the need for home health services.
  3. Home health care is available to those in need of care that requires the skill of a nurse or therapist. Care is based on goal-centered progress through shared education. Compliance with the care plan and a willingness to learn about disease management and skills for improvement should be present.
